Accessible Textbook Request Form - Student Disability Center

This form is for students who have already been approved for accessible textbook accommodations. If you would like to discuss receiving textbooks in an accessible electronic format please make an appointment with an accommodations specialist by contacting our office.

Please submit this form every semester that you would like your textbooks provided in an accessible format. You can find out what textbooks yo uneed by visiting the CSU Bookstore website.
