This form is only for returning SDC students to reconnect with the SDC office. Completing this form will reauthorize your accommodations for the semester, such as extra time on exams, note taking support, flexible attendance or alternative format for text.
If you want the same accommodations you’ve had last semester implemented again, complete this form. Your accommodation letters will be generated and will be available for you to pick up at the front desk of the SDC, 121 TILT. It is your responsibility to deliver the letters to each of your instructors BEFORE you will be able to utilize the accommodation (such as scheduling an exam). You will be notified when your accommodation letters are available.
If you feel you need to change your accommodations, you will need to make an appointment to see one of the SDC accommodation specialists to have the change approved and implemented.
