This form is only for SDC students who have already set up accommodations. Completing this form will reauthorize your accommodations for the semester, such as extra time on exams, note-taking support, flexible attendance or accessible format textbooks.
Your accommodation letters will be generated and emailed to your professors with you copied on the email. It is your responsibility to request the letters BEFORE you will be able to utilize the accommodation (such as scheduling an exam).
If you feel you need to change your accommodations, you will need to make an appointment to see one of the SDC accommodation specialists to have the change approved and implemented.
