The below form is for students who have scheduled an appointment to meet with the Student Disability Center. If you have not scheduled an appointment, please contact our office to schedule an appointment.

Before your appointment, you can submit the below form to provide the SDC with information related to your accommodation request. Depending on what you share, some accommodations may be able to be approved before your scheduled appointment. If that is possible, an accommodation specialist will contact you about this.

We encourage you to share as much information as you feel comfortable, as this can help us better understand your request. Providing thorough information and documentation may help the accommodation determination process move as quickly and efficiently as possible.  We will keep your information as private as possible. Our office follows privacy guidelines set forth by FERPA. We won’t share any information with your family or anyone outside of CSU without your consent. We may need to share some information with relevant faculty and staff within CSU, such as your instructors. We keep the information we share as minimal as possible and focus only on what’s relevant to the situation. We are obligated to report any information regarding harm to self or others. We also have to report any information related to interpersonal violence or sexual assault to the CSU Office of Title IX Programs.
